  • Family Genogram Information

Providing this information is an important step in completing a Family Genogram that your therapist will help you use to gain insight into how your family patterns may be impacting you now so that you can be empowered to make changes that you want.
  • Think about three generations of your family starting with your grandparents, moving to your parents, and then to you and your siblings, cousins, and whoever else you view as family.  For each person, give as much information as you can and list everyone in your family, living or not.

Relevant information would include living or dead, profession, health conditions (including mental health), married/divorced, and any thing you find significant.

Include miscarriages if you are aware of them. For age, your best guess is fine and if a person has died, please put your best guess of how old they were when they died.
